In the United States, the current format for organ donation is opt-in.  This means that someone has to actively volunteer to be an organ donor and, in reality, their family or decision maker must also consent upon their death. Given the tremendous shortage of organs for transplant in this country, many have proposed different versions of an opt-out policy (i.e. a policy that presumes upon death everyone is an organ donor unless otherwise stated).  Others have proposed a middle ground between these two options such as requiring everyone to declare their decision to either opt-in or out of organ donation each year when filing their taxes.
