Service- Learning Paper Project Rubric
IPE Team: I was the social worker, we had a nurse, psychiatry doctor, a physician and a dentistry, as the patient had a Dementia.( Spent 3 ½ hours)
Spent 6 hour talking to my adapt Grandparent. Name of the agency is whenwecare.
During the semester, you were required to provide at least 10 hours or more of service with the UTHealthInterprofessional (IPE) collaboration event (3 ½ hours), Adopt a Grandparent Program (4 to 5 hours) (1 hour and 30 minutes -optional) You will then prepare a paper based on your experiences for the 10 hours, which incorporates key social work concepts and principles from the course. Given that the paper is based on a completed learning experience, you may not receive credit for an incomplete project. Therefore, failure to complete the required 10 hours will automatically disqualify you from proceeding with the Service Learning Project.
Paper formatting instructions: When preparing your paper, please use APA 6thor 7th edition formatting (i.e., title and reference page; double spaced; 12 pt. font (even for the title) in Times New Roman; double-spaced pagination; and 1.0 inch margins on each page, without bolding or color). No abstract page required for this paper. Do NOT submit an abstract.
Paper content instructions: In 4 to 5 pages (not including title or reference pages), please provide the following information regarding your service:
PART I
Service -Learning Experience
- In 1 page: Provide the basic information about event, program, or webinar where you spent your 10 hours to include: who they are and what types of services they provide to the community, who they serve in the community, where they are located, and the basic history of the event or program. In addition, please provide the name of your contact/supervisor in the agency (including how you contacted them).
- In 2-3 pages: Provide a very brief description of exactly what you did during your 10 hours in the agency. Based on these experiences, you should then provide detailed reflection and discussion on the following prompted questions:

- How does the even or program services impact older adults?
- What impact does the event, program services, webinar have on the larger community within which it operates? (Engaging in Regional, National & Global Communities);
- Compare and contrast your worldview (i.e., values, beliefs, and practices) to the mission, vision, and services of the event, program, webinar (Intercultural Competence)
- As a result of your service- learning project, what have you learned about your thinking and values as they relate to older adults in the larger community? (Civic Responsibility)
- What are your parting thoughts on your experience working with older adults? Discuss any skills, values, knowledge, perceptions, attitudes that you currently have about this population.
- Did you experience any emotions that impacted your judgement (affective processing) working with this population?
- How did you use critical thinking (cognitive processing) when working with this population? Give examples
PART II
Reflection Portion of the Service-Learning Paper
In 1 page or a page and a half:Provide a reflection of your service-learning experience with the IPE event, the Adopt a Grandparent .
Paper Elements (and proposed structure of reflection):
- What values, skills, and knowledge did you gain working your service-learning hours with the IPE event, the Adopt a Grandparent program?
- What part of the service-learning did you enjoy the most? The least?
- How do you think your service-learning experience with older adults will assist you in working with older adults as a practitioner or in general?
- If you could share with others about your service-learning experience with others, what will you share with them?
- Have your attitudes or perceptions changed toward older adults after this service-learning experience? If so, how?
