Part 1: (Roughly 600-750 words) 

Note: You may assume you are clinicians or case managers for this exercise.

Write a one-page professional disclosure statement and informed consent describing the following: (I HAVE UPLOADED AN EXAMPLE) 

1. Your training and expertise: This can be fictitious 
2. What activities the student will conduct during the sessions. 
3. Who your supervisor is, and how they can be contacted: This also can be fictitious.  
4. The possible downside to the prescribed treatment. This usually includes potential reactions in therapy. For example, what might a trauma victim experience when discussing their trauma.  
5. You may use first person when writing this assignment. An example has been provided for you in the topic materials.

Part 2 (Roughly 600-750 words) 

          The second part of the assignment asks for you to explain why informed consent is important. When explaining this please include some references. Also, it asks about litigation examples. If you can find them, good, but if you can’t find examples, that’s okay as well. Just include a clear understanding with the assistance of references to support why informed consent is important.
