This paper is a made-up case study of a “patient” who suffers from Lyme disease. The professor wanted us to pretend we were a neuropsychologist meeting a patient with Lyme disease and provide a very thorough:-background on the disease including symptoms, brain pathology, genetic links, neuropsychological performance, prognosis/rehabilitation strategies, and success-patient introduction: reason for referral, the presenting problem(can include functional limitations), medical/psychiatric history, family history, educational/occupational history, social history(married? children? what do they do for fun?), behavioral observations-patient background including the reason for visitation; types of symptoms; what the caregivers describe as symptoms (if accompanied by one); medications (if taking any); family medical history; patient’s appearance/behavior; job/home life/ history-medical/neurological findings (did the patient’s medical tests reveal anything of significance? could be a blood work test, physical, brain scan, genetic testing, etc.)-neuropsychological assessment including what tests were administered and how the patient did on them, what abilities seemed normal vs. deficient, etc. MUST PROVIDE SPECIFIC TESTS USED.
